The CEPPP is collaborating with the Canada Research Chair in Partnership with Patients and Communities on the Caring Community research project. This social innovation focuses on the synergy between the community and the health care system, capitalizing on the capacity of patients and citizens to create social ties and care for one another.

The goal of this project is to develop patients’ caregiving skills, break social isolation, foster collaboration between different caregivers in the community and encourage recovery towards full citizenship. These objectives are achieved by integrating peer mentors into care teams and the neighbourhood, and by mobilizing community, health and municipal decision-makers to create environments conducive to social support.
